2. Complete and sign the form and send to Birth Control Buzz with your prescription(s) toll-free to 1-866-732-0306.
Your prescription(s) will be verified by Birth Control Buzz with your physician's office. We also require a copy of one of the following: driver's license, passport, military ID, or birth certificate.

3. A Healthcare Specialist will contact you to confirm your order.

4. To complete your order we ask that you also mail your original prescription to our office -
